Pay Rate: $1808.00 - $1992.00
Required QualificationsHome Health and Oasis experienceEvergreen's
minimum booking requirement is 25 days outEvergreen is requiring
all employees and contract staff to have the Covid and Infuenza
vaccine. Covid - The last day for full vaccination is Oct 18, 2021
defined by the CDC as "two weeks after completion of the last dose
of vaccine."Infuenza - no later than Nov 22, 2021Per communication
with all employees, there are only 2 options to opt out:"Those of
our colleagues who decline the vaccine for medical or religious
reasons will not be subject to criticism or penalized in any way.
We WILL require the completion of a formal declination form and
confirmation in writing from the individual's physician or
religious leader. Personal preference declinations will not be
accepted."
